What is Akoua?

Akoua is a private, Australian AI medical scribe. It listens to a consented consultation, transcribes it in near real time, and drafts a structured note from only what was said — which you review and confirm before anything is saved.

It is a documentation aid, not a diagnostic device. It makes no clinical inferences.

What it does during the consult

• Transcribes in near real time — words appear on screen as the conversation happens.
• Leaves speakers unlabelled while you talk — the live transcript is unattributed, and Akoua works out who spoke after capture rather than guessing in the room.
• Drafts the note from the transcript — SOAP by default, or your template.
• Marks what’s absent — anything not said is marked [not stated], never padded with plausible filler.
• Flags what it isn’t sure of — a dotted underline means “not sure I heard this”; a highlighted block means “heard it, couldn’t place it”.

That last part is the difference worth knowing about: a confident error is the one you sign without noticing. Akoua flags what it isn’t sure it heard and what it couldn’t place cleanly, so your attention goes straight to the few spots that need it.

Nothing saves itself

Every model-generated draft requires your explicit review and confirmation before it is saved or exported. A consequence-ranked review queue puts the riskiest flags first, and nothing is committed on the model’s own. You remain responsible for the accuracy and completeness of the final note, exactly as you are today.

When you confirm, the audio Akoua holds is deleted — it is kept encrypted only while you review, so you can listen back to anything flagged — and the patient can keep a QR deletion receipt.

After the consult

Speaker attribution is added here rather than during the consult: speaker labels appear only in the final transcript, and only where the available evidence supports them, and when Akoua isn’t sure who spoke it says so rather than mislabelling.

Referral letters and dated tasks from spoken follow-ups come from the same consult, and each one is a draft you confirm. For Australian organisations, MBS item-number hints come from it too, but they are read-only suggestions Akoua never saves, exports or submits, so they carry no confirm step; the hints are Australian-only and switched off everywhere else. Notes and letters export as clean text and PDF/A that you paste or attach; Australian practices can connect Cliniko or Nookal today; other practice-software integrations are on the roadmap, and we’ll say “integrated” only when it actually is.

Akoua is open in Australia and New Zealand, and the United Kingdom service is open too. Outside those regions, clinical access is coming soon — register interest for future access rather than creating an Australian account.
